FILAC and complex anal fistulas: a good alternative to surgery

Laser therapy in proctology is a fairly recent technique which treats anal fistulas (FIstula-tract LAser Closure: FilaC®). This procedure allows the sealing of fistula, which involves the "burn" radially with the laser without damaging the sphincter. We report the case of a 46 years old patient, operated several times for complex anal fistula. She was admitted to our department for anal abscess complicating a recurring horseshoe anal fistula. The first operating time allowed drainage of the abscess with the placement of 2 setons (A). The anatomopathological and bacteriological study (culture) of the anal fistula tract as well as the search for mycobacterium tuberculosis DNA by PCR (GeneXpert) made it possible to exclude the diagnosis of anoperineal tuberculosis. The patient was reoperated 5 weeks later for the completion of a minimally invasive laser treatment (FILAC) which enabled the closure of the two drained fistulous tracts with an accessory path discovered during the 2nd operating time (B, C, D). The intervention took place under spinal anesthesia. The postoperative recovery was simple with good healing without recurrence after 9 months follow-up. FILAC is a minimally invasive conservative intervention, the postoperative management are marked by a significant reduction in pain, this technique constitutes an interesting therapeutic option for the treatment of complex fistulas.

La Laser-thérapie en proctologie est une technique assez récente qui permet de traiter les fistules anales (FIstula-tract LAser Closure: FilaC®). Cette intervention permet l´obturation du trajet fistuleux, qui consiste à le « bruler » de manière radiale avec du laser sans endommager l'appareil sphincterien. Nous rapportons le cas d´une patiente âgée de 46 ans, opérée à plusieurs reprises pour une fistule anale complexe. Elle a été admise dans notre service pour abcès anale compliquant une fistule anale en fer à cheval multirecidivante. Le 1er temps opératoire a permis un drainage de l´abcès avec mise en place de 2 élastiques (A). L'étude anatomopathologique et bactérologique (culture) du trajet fistuleux ainsi que la recherche d´ADN de mycobactérium tuberculosis par PCR (GeneXpert) ont permis d´exclure le diagnostic d´une tuberculose anopérinéale. La patiente est reprise 5 semaines après pour la réalisation d´un traitement mini-invasif au laser (FILAC) qui a permis la fermeture des deux trajets fistuleux drainés avec un trajet accessoire découvert au 2e temps opératoire (B,C,D). L'intervention s'est déroulée sous rachi-anesthésie. Les suites post-opératoires étaient simples avec une bonne cicatrisation sans récidive sur un recul de 9 mois. Le FILAC est une intervention mini-invasive conservatrice dont les suites opératoires sont marquées par une réduction significative de la douleur, cette technique constitue une option thérapeutique intéressante pour le traitement des fistules complexes.
